If submitting electronically:
- Please fill out the Photo Release Form
- Email it and a picture of the baby to kim.desolis@maygrant.com
If submitting through the mail:
- Please print and fill out the Photo Release Form
(Note: the form is also available at our office if you do not have access to print)
